Stock Gift Form

This form allows you to declare your intention to make a publicly traded stock gift to Advent Episcopal School. To make the transfer of your gift as easy as possible, we have a three step process:

  1. Complete the informaton requested below. (You may want to keep a copy for your records).
  2. School will contact you with the transfer instructions after we have received your completed form.
  3. You will need to provide the transfer instructions to your broker.

The information collected in the form will allow Advent Episcopal School to credit your gift appropriately and in a timely manner.

This specific form allows you to designate your gift to one of the four funds listed below. You will be asked to select the fund where you would like your gift to be designated.

Una & Craig Battles Innovation Fund
Rick Phillips Community Service Fund
Memorial Gift
